art time is 8:30 AM. Please have your paperwork filled out before arrival. Three forms must be filled out and completed. Parents, please make sure that you sign all 3 forms. Also, we must have your insurance company and policy number. Here are the following 3 forms: 1. Consent and Release from Liability form = Please make sure to fill out Insurance policy number and sign the form 2. Consent and release from Heat and Concussion form = please make sure to sign. 3. Actual Physical Form = Please fill out all Information and sign the form.

**August 6th, Monday: The start of Football Practice (Camp) for all classes. Practice starts at 7:15 AM. Camp will end at 1:00 PM. You must have completed all three forms with parent signatures, insurance and policy numbers to participate!!

**August 8th, Wednesday: Mandatory Parent Meeting at 6 PM in the Auditorium.

**Additional Paperwork: Pay to play forms must be filled out and the money turned into the Athletic Director before the Lake Mary Game or players will not be allowed to play. Please take care of this during the first week of School!!
