ATHLETIC CODE CONSENT FORM

I have received, read, and understand a copy of the "WCUSD #2 ATHLETIC CODE", I understand that the purpose of this athletic code for Westville High School is to assure the physical, mental, and moral well being of all student athletes.  The administration and athletic department will adhere to the stated regulations for all sports for boys and girls, including  cheerleading and dance, throughout their entire high school athletic career.

**This athletic code consent form will be in effect during the student/athlete's high school eligibility and will be kept on file during the student's high school years.
