Physical Information

Parents,

Athletic Physicals will not be offered at school as in the past years.  However, Nurse Allbaugh has set up times with area clinics in order to offer our athletes physicals at prices the same or similar to those offered last year.  Physical forms will be sent home with your student.  Your part on these should be filled out ahead of time.  Please see the schedule for the participating doctors below. 

Parents should also check with their family doctors as some will complete the sports physical paperwork without another doctor visit if the student has been seen in the past calendar year for his/her well-child check.
