Sixth Grade Shot Info Just a Reminder, Each student entering the 6th grade in the upcoming school year is required to have a physical and immunizations update. To obtain these, you may schedule an appointment at the Johnson County Health Dept. (789-2590) or at your physician's office. A sport's exam can count as a physical, however, a 6th grade physical isn't sufficient for a sport's exam. Immunizations required by the state are Tdap or Tetanus, Varicella (Chicken Pox) Booster or documentation from your health provider of having the Chicken Pox disease and Menactra (Bacterial Meningitis) Vaccine. The Gardasil (HPV) Vaccine is also highly recommended for girls and boys 9 years of age or older. You can receive more information from the school nurse (789-2651), the Johnson County Health Dept. or your health care provider regarding these requirements. The physical form and updated shot record must be turned in to the nurse at the beginning of the school year.
