IHSAA Sports Physicals

All fall athletes will need a new physical on file at the athletic department office by July 30th to be able to participate in first day of practice on the 30th (Girls' Golf 7/27/12). Forms can be printed off from Recent Documents below or you can pick a form up at school from the slotted rack beside the Athletic Department bulletin board. This includes all Junior High and High School athletes.
