Mandatory Fall Parent Meeting Information

On August 9, 2012 all LC parents who have a son or daughter in a fall sport were  to attend a Mandatory Parent Meeting at 6:00 p.m. in the HS Auditorium. For those parents who had an extenuating circumstance, you will need to do the following.

1. Watch the parent meeting video on this website (will be posted the morning of August 10, 2012.) Parent must then sign the Parent Meeting Signature Form under the Parent Meeting Forms tab on left side of this website. By signing you confirm that you watched the on line video version of the meeting. The form then must be turned in to your coach or the athletic department office.

2. The Parent and Athlete must read the Heads Up Parents and Heads Up Athletes information sheets on concussions found under the Concussion Information tab on the left side of this website.

3.The Parent and Athlete then must sign all proper locations on the "Blue Form" found under the Parent Meeting Forms tab on the left side of this website, and turn in to your coach or to the athletic office. 

Without a signed Blue Form, and Parent Meeting Signature Form the student athlete is not eligible to compete in athletics.
