Session 2 – July 17^th 5:30 pm – 7:00 pm (Quickness, Speed, Strength, Power, Plyo’s) $30

I, the undersigned parent or guardian of the above student, understand that Josh Niblett Speed Camp insurance will cover only those costs over and above the existing health insurance I am providing for my Camper. I authorize the Camp Staff to act in their best judgment should a medical emergency occur that affects my Camper. I hereby release, discharge and/or otherwise indemnify the (Organization), all Board members, coaches, its affiliates, organizations and sponsors, their employees and associated personnel, including the owners of the fields and facilities used for the programs, against any claims by or on behalf of the registrants as a result of the registrant’s participation in the programs and/or transportation to or from the same, which transportation I hereby authorize.
