Letter from the Athletic Director

Dear Prospective student-athlete and family;

Henry Clay High School Athletics is governed by the Fayette County Athletic Guidelines and the Kentucky High School Athletic Association, KHSAA.  The KHSAA is a branch of the Kentucky Department of Education.  In order for any student to try-out for a high school team there are several state and Fayette County guidelines that must be followed.  Listed below are the items that prospective student-athletes are required to have before the student is permitted to try-out for any athletic program at HC.

The Parent Must:

  1. Read pages 1-6 of the KHSAA guidelines that are part of the physical*
  2. Complete pages 7, 8, 9 & 10 of the KHSAA physical (page 8 will be filled out by the physician)
  3. Sign the following forms:
    a. HCHS Drug Test Permission
    b. HIPAA*
    c. FCPS Media Release*
    d. FCPS Transportation*
    e. HCHS Address verification*
  4. Pay $37.84 for athletic insurance required by all participants of athletics in Fayette County (may go change this school year)
  5. Pay $14 for drug testing

The Student Must:

  1. Provide the athletic information on Part 1 of the KHSAA physical - Athletic Information pg. 7
  2. Read and sign the Athlete's Acknowledgement of Risk; Part V, KHSAA physical - pg 10
  3. Must have a copy of a CURRENT physical in hand the day of tryouts to participate - a physical is good up for ONE year (NO EXCEPTIONS)
  4. If TRANSFERRING into Henry Clay, fill out transfer paperwork with the Athletic Director. Students who transfer are ineligible until transfer forms are received from the sending school and approved by the KHSAA,

Having all of these items ready prior to tryouts will assist the coach and the athletic department due to the requirements set forth by FCPS and KHSAA.
