SUMMER CAMP INFORMATION

PRETZEL ATHLETICS INFORMATION

Every athlete needs to meet the participation requirements yearly. The athletic participation requirements include: 1- Must have a valid physical on file. 2- Complete all required paperwork. 3- Must have passed at least 5 classes during the previous semester. 4- Must not have any outstanding fines or obligations from a previous activity. 5- Must have attended a code of conduct meeting with a parent/guardian prior to his/her participation in the first activity. Once all of the requirements are met, the coach will receive a pink eligibility card which enables the athlete to participate.

The required paperwork is listed below. The forms can be accessed below by clicking on the title. Print out the forms, complete them and bring them to an athletic code meetings. Both the parent/guardian and athlete are required to attend one meeting per year prior to the first activity of participation.

- Tuesday, August 6 -- Poms, Cheer, Girls Tennis

- Wednesday, August 7 -- Boys Soccer, Boys & Girls Golf, Girls Swim

- Thursday, August 8 -- Volleyball & Cross Country

- Friday, August 9 -- Football

- Friday, August 16 -- Makeup Meeting

PHYSICALS

- All athletes must have a physical that is valid for the entire season.

- Physicals can be in the form of a sports physical or a complete medical exam.

- Physicals are valid for a period of 395 days from the date of the physical.

- All incoming freshmen need a complete medical exam.

Sports Physicals @ FHN Burchard Hills -- $20.00

- Saturday, May 11 ----- 10:00 a.m. - 3:00 p.m.

- Thursday, June 6 & Monday, August 12 ---- 5:30 - 7:00 p.m.

- Sign up for an appointment @ FHS Business Office 815-232-0425. After June 10th call 815-599-7440 for

the August appointment.
